A Personal Representative (PR) is an individual who may represent a Consumer with a disability or medical need in regard to personal assistance services. A PR may be designated by a consumer who chooses not to manage their own care; or may be designated on behalf of a consumer who has been deemed ineligible to direct their own care. Any individual acting as a Personal Representative must be screened by Mountain Pacific Quality Health and meet the standards set by the State of Montana. The PR is responsible for recruitment, hiring, training and managing of the Consumer’s personal care attendants. The PR must meet with the SDPAS Program Team and Mountain Pacific Quality Health when deemed necessary. The PR is further responsible for approving and signing all Service Delivery Records (timesheets).
