The Plan of Care includes all the signed forms and agreements needed to authorize a Consumer’s participation in the Self-Direct program.

- Glossary: Individual Cost Plan [ICP]a summary of costs, units of service and funding sources relevant to a person’s plan of care.
- Glossary: Individual Family Service Plan [IFSP]the plan of care developed for a child ten to sixteen years of age in Developmental Disabilities Children’s Services, which documents the habilitative actions and outcomes the family wishes to achieve in a specific time frame.
- Glossary: Personal Support Plan [PSP]Personal Support Plan, the annual plan of care used for individuals sixteen years of age and older in Developmental Disabilities Program Services which documents the individual’s assessed needs and desired outcomes, detailed in the actions which are documented in the plan within a specific time frame.
- Glossary: Rountines-Based Interview [RBI]Routines-Based Interview, a clinical, semi-structured interview process designed to establish a positive relationship with the family and obtain a rich description of the child and family functioning. A parent or other adult functioning as a parent is always interviewed. If the child is cared for by someone else for fifteen hours or more a week, this person is also interviewed. The interview assesses the child’s engagement, independence and social relationships within everyday routines and can be used to obtain a narrative description of the child’s cognitive, motor, adaptive, communication and social skills; it does not result in a score but helps to determine the families satisfaction with home routines, resulting in a list of outcomes chosen by the interviewee and is used in the family plan of care development
