The Veteran Directed Care (VDC) Program is designed to give you more control, flexibility, and independence in planning the support you need at home. With a flexible budget, you can choose the caregivers who know you best—whether that’s a family member, a friend, or someone you trust—and customize your care to fit your daily life and personal needs.
At Ability Montana, we believe you deserve to live life on your own terms. Through the VDC Program, you decide what kind of care you need, who provides it, and how it supports your independence. Services may include help with personal care, household tasks, and other assistance that allows you to stay in your home and remain connected to your community.
Be enrolled for services with the Veterans Health Administration
Have a VA primary care team at your local VA Medical Center
Meet the eligibility criteria for the Home and Community Based services as determined by the VA
Receive a referral from the VA Medical Center to Ability Montana
